Androgenetic alopecia causes a shrinking of the hair follicle in certain areas and a shortening of the growth cycle. With each succeeding cycle, atrophy (shrinking) of the hair follicle takes place, leading to hair thinning. When boys reach puberty, they produce more testosterone, a virilising hormone that prepares them for a sexually active life. Testosterone, produced in the testes and adrenal glands, changes the vellus hairs on his chin, pubis, and auxilla to terminal, while if sensitive to, at the same time, gradually changes the terminal hair on top of his head to vellus (finer) hair (the effect of a more potent hormone called dihydrotestosterone on DHT sensitive follicles). This causes slow atrophy and shortening of each successive growth cycle until the inherited pattern has occurred.

This describes the sudden appearance of a bald patch or multiple patches. In some cases, a variant ophitic form develops around the margins of the head. To start with, there can be irritation before the patch occurs. Around the periphery of the patch, there are very short stumps of hair; these are called exclamation hairs and, under the microscope, show where the hair shaft has been weakened by autoimmune dysfunction. Associated signs can include sudden loss of pigment in a patch on the skin; this can happen anywhere on the body. The atopic state (Asthma, Hayfever, and Eczema) is a common co-factor. Trichological treatment is offered for this condition; its prognosis can be unpredictable, and a variable approach to treatment may be required. If nutritional therapy is needed, patients are advised that compliance is paramount to a successful prognosis.
Alopecia universalis is a rare and severe form of alopecia areata, which is an autoimmune disorder that affects the hair follicles. In alopecia areata, the body’s immune system mistakenly attacks the hair follicles, leading to hair loss. In the case of alopecia universalis, this autoimmune response is particularly aggressive and results in the complete loss of all body hair, including the scalp hair, eyebrows, eyelashes, and even body hair.
Alopecia totalis is a subtype of alopecia areata, which is an autoimmune disorder that affects the hair follicles. In alopecia areata, the immune system mistakenly attacks the hair follicles, leading to hair loss. Alopecia totalis is characterised by the complete loss of scalp hair, resulting in baldness across the entire head. Unlike alopecia universalis, which involves the loss of all body hair, alopecia totalis primarily affects the scalp.
There are many causes of scarring alopecia, and they are often only detected once the condition is well established, as in the photo to the right taken on the patient’s first consultation. Early detection and intervention is crucial. Those who suffer from scarring alopecia are advised to take regular checkups until the underlying condition has passed. Some examples are Folliculitis Decalvins, Lupus Erythematosus, Pseudopelade, Acne Necrotica Milliaris, and Acne Keloidalis, which, if not treated and managed, will result in scarring.
